Florida Health Solution Corp. (FHS) is a Florida licensed Prepaid Health Clinic & Discount Plan Organization: The corporate name and location : Florida Health Solution Corp. 7350 NW 7th St. Suite 204 Miami, FL 33126.

*PHC (Prepaid Health Clinic): Florida Health Solution Corp. is a Florida licensed Prepaid Health Clinic (PHC). FHS pays for services rendered at participating providers, members will pay a minimal copayment when receiving PHC healthcare services. Preventive panels at no cost to member, plan benefits are available based upon eligibility.

**DPO (Discount Plan Organization): This plan is not insurance. This plan provides discounts at certain health care providers for medical services. This plan does not make payments directly to the providers of medical services. The plan member will pay for all health care services at a negotiated rate.
